What should be reported and why

The things listed below should be written on the Record of Visit each time they occur and reported to your line manager or group leader:

  • Threats and intimidation
  • Presence of any weapons
  • Attacks on staff
  • Abusive telephone calls
  • Attacks on service user or visitors
  • Substance abuse
  • Self injurious behaviour
  • Damage to property
  • Near misses

These are the reasons why we report the incidents listed above:

  • To comply with Health and Safety Regulations
  • To highlight developing problems
  • Highlight resource issues
  • To monitor the effectiveness of policies, procedures and care plans
  • Provides protection for service users and staff
  • Highlight the frequency of interventions, staff involvement and training requirements
  • Highlight any developing patterns or sequence of events
