The prescribers name, practice address, telephone number and job role
The prescribers signature
The date the prescription was issued
The client’s full name, address and date of birth- for children under 12 their age should be specifically noted
The generic name of the prescribed medicine
The route which the medication should be taken
The dose of the medication and frequency it should be taken
If the medication is to be taken ‘as required’, the prescription should specify a minimum dose interval
